PHYSICAL, IMMUNIZATION AND ATHLETIC INFORMATION

Mandated Physical Examinations

Since July 2018, New York State public school students must have a health exam when they enter school as a new student and in grades Pre-Kindergarten (Pre-K) or Kindergarten (K), 1, 3, 5, 7, 9 and 11. The examination should be completed on the New NYSED Student Health Examination form. Please return the completed form to your school nurse.

Medication in School

Medication administered at school must be carried into the nurse's office by an adult. The medication must be in the original container. The medication must be presented with the medication form (link to form) completed by the MD and signed by the parent. Same rules apply for over-the-counter medication.


Asthma Management

All schools have a nebulizer in the nursing office. If your child requires nebulizer treatment, please provide the nurse with MD order, medication and the tubing with the mouthpiece.

Students with asthma may carry their rescue inhaler with them if the nurse has an MD order on file indicating this is okay. We strongly recommend a back-up inhaler be housed in the nurse's office. Please tell your child to notify the nurse early if they are using the rescue inhaler more than once in the day so that the nurse can monitor his/her lungs and avert a crisis.
