Program Evaluation Tool (Replacing textbook eval 6-19)
This form should be submitted at the completion of a pilot when requesting a district adoption. Committees should supply this form to individuals involved at any level of the program evaluation. A summary of all forms should be provided to the Curriculum Advisory Council. Individual copies should be stored with the committee co-chairs and may be requested by the CAC.
