I acknowledge that:
- Funds raised will comply with State Education requirements and must be deposited and accounted for as per district guidelines.
- All activities must align with district policy 1510.
- Food sold to students for fundraising efforts, and/or food sold to students that is not a part of the approved school lunch program during the school day is strictly prohibited.
- The fund drive will include a training program for the solicitor. A training document, 1510-E, will be distributed to the fundraising supervisor for each activity.
Note, if a flyer has been developed to advertise the activity, including but not limited to a flyer describing apparel, that flyer must accompany the submission of this form.
