NEW STUDENT REGISTRATION/CHANGE OF ADDRESS*
PARENT OR GUARDIAN, PLEASE BRING PHOTO I.D. ALONG WITH THE FOLLOWING:
  1. BIRTH CERTIFICATE – (ORIGINAL COPY WILL ONLY BE ACCEPTED FOR KINDERGARTEN)
  2. IMMUNIZATIONS – Proof of mandatory immunization for DPT, POLIO and HEPATITIS B (series of 3), 2 MMR (administered AFTER one year of age) or 2 MEASLES, 1 MUMPS AND RUBELLA.   All children born on or after 1/1/1994 must have a VARICELLA shot, administered AFTER one year of age.  TDAP is needed for all students born on/after 1/1/94 and entering 6-9th grades. Pre-school children must have DPT, POLIO and HEPITITIS B SERIES, 1 MMR (after 1st birthday), 3 shots of HIB or one shot given after 15 months of age and 4 shots of PCV7. A certificate indicating the results of Lead testing should also be presented upon registration.  ALL IMMUNIZATION DATES ARE MANDATORY AND MUST HAVE A DOCTOR’S SIGNATURE. 
  3. PLACEMENT RECORDS – Elementary students require a transfer card, or report card.  Special Education Students require a copy of IEP (Individual Educational Program).  Secondary students require a transcript of grades and courses completed.
  4. *RESIDENCY PROOF – All parents or legal guardian registering students must be residents of the Brentwood School District.  For resident students to attend District schools, THREE proofs of residency shall be required as follows:  A mortgage or closing statement, a deed, a notarized signed lease, or a current notarized rent receipt (with address written on receipt) for a home in the District and any TWO of the following:  An original bill stating residency, Driver’s license/DMV I.D. card, Bank statement, Voter registration card or DSS declaration (postmarked no more than 30 days of registration).  In addition, homeowners will need a notarized affidavit of legal residence.  Rental tenants will need notarized affidavits of residency completed by both the tenant and homeowner.
  5. GUARDIANSHIP CASES – Anyone registering a student, except the parent(s) must meet with the Coordinator of Census and Attendance and present evidence of guardianship.
  6. FOSTER CASES – All foster parents must present at the time of registration of a foster child, a placement letter from the agency, on their letterhead indicating the foster parent’s name plus date of birth and grade of each student to be registered.  Agency must also include health history and dates of mandatory immunizations of each student, and a DSS 2999 form with CIN/Medicaid number, ACS or SOO number for each student.  Only agency caseworker can register Special Education students.
  7. STEPPARENTS – Must present at the time of registration a copy of their marriage certificate.
  8. HOURS – The registration office is located at 795 Wisconsin Avenue, Special Services Building (red building next to the Oak Park School) and is open Monday through Friday from 8:00 a.m. to 2.00 p.m.
  9. The Homeless Liaison located at Central Registration will facilitate and expedite registrations for families residing in temporary housing situations (i.e. shelters, self-declared, unaccompanied youth).
  10. Testing or evaluation appointments may be necessary at another day.
  11. If there is a problem that cannot be resolved, please make an appointment to discuss the problem with an attendance staff member, at 434-2301.

NUEVA REGISTRACION DE LOS ESTUDIANTES/CAMBIO DE DOMICILIO*
PADRES O GUARDIANES, FAVOR DE TRAER IDENTIFICACION CON SU FOTO JUNTO CON LO SIGUIENTE:

  1. CERTIFICADO DE NACIMIENTO – (ORIGINAL SOLAMENTE PARA KINDERGARTEN)
  2.  INMUNIZACIONES (VACUNAS) – Prueba de inmunizacion obligatoria contra la DPT, POLIO y HEPATITIS B (series de 3), 2 vacunas contra el (MMR) Sarampion, Rubiola y Paperas (Administrada DESPUES de un ano de nacido). Los ninos de escuela pre-escolar tienen que tener 3 vacunas contra HIB o una vacuna administrada despues de 15 meses de nacido y cuatru vacunas de PCV.  Ninos nacido 1/1/98 o despues y ninos nacido 1/1/94 o despues necesitan una vacuna contra la Varicella administrada despues de un ano de nacido.  TDAP es necesario para todos estudiantes en el grado 6 y 9.  ES NECESARIO TENER LAS FECHAS DE TODAS LAS VACUNAS ESTAMPADO CON LA FIRMA DEL MEDICO.  Un certificado indicando los resultados de veneno contra el plomo, tambien debe de ser presentado.
  3. DOCUMENTOS NECESARIOS PARA LA PROGRAMACION DE LOS ESTUDIANTES – Alumnos de la escuela primaria necesitan presentar una tarjeta de cambio de escuela o una libreta de calificaciones.  Los ninos de Educacion Especial necesitan un I.E.P. (Programa Educativo Individual).  Estudiantes de la escuela superior necesitan una copia de los cursos completados y las calificaciones.
  4. *PRUEBA DE RESIDENCIA – Todos los padres o guardianes legales quienes registran los alumnos deben ser residentes del distrito escolar de Brentwood.  Para que un estudiante residente pueda asistir a las escuelas del Distrito, TRES pruebas de residencia son requeridas de la siguiente forma:  Hipoteca de la casa o escritura de cierre, deed, un contrato firmado y notariado, o/un recibo actual de renta corriente notariado, (con la direccion escrita en el recibo), para una casa en el Distrito, y DOS de cualesquiera de las siguientes:  Recibo de Impuestos de Suffolk County, Recibo de Telefono, Recibo del Agua, Recibo de la Compania de Aceite/calefaccion, Recibo del Seguro, Licencia de Manejo/Identificacion, Recibo del Estado de Cuenta Bancario, Tarjeta de Registracion para Votar, (marcada postal no mas de 30 dias de registracion). Agregar los siguientes documentos, duenos de casa necesitaran una declaracion jurada y notarizada (affidavit), de su residencia legal.  Inquilinos necesitaran una declaracion jurada y notarizada de su residencia legal, esta tiene que ser completada por el arrendatario (dueno) y por el inquilino.
  5. CASOS DE CUSTODIA – Es debido que los custodios o guardianes legales hagan cita con el coordinador de esta oficina antes de la matricula.
  6. CASOS DE ADOPCION TEMPORAL – Todos los padres que adoptan ninos temporalmente (FOSTER PARENTS) deben presentar al momento de la matricula una carta de la agencia que indique el nombre de los padres adoptivos temporal, la fecha de nacimiento del alumno y el grado correspondiente.  La agencia debe ademas incluir un historial de la salud del nino/a y las fechas de las inmunizaciones.  El formulario DSS 2999 y otras informaciones son necesarias.  Solamente la trabajodora del caso puede matricular estudiantes que asisten el Programa de Educacion Especial.  Si necesita informacion sobre los requisitos haga el favor de llamar por telefono a esta oficina.
  7. PADRASTOS – Es necesario presentar el Certificado de Matrimonio en el momento de las matricula. 
  8.  HORARIO – La oficina de matriculacion esta localizada en el edificio de Servicios Especiales en la Avenida Wisconsin    (edificio rojo junto a la escuela Oak Park).  Las horas son de 8:00 a.m a 2:00 p.m. de Lunes a Viernes.
  9. La Union de Enlace para Desamparados localizada en la Oficina Central de Matriculas, le facilitará y expedirá matriculas a familias que residan en albergues temporarios, (I.e. albergues, refugios, declarado por si solo, o joven sin acompañamiento).
  10. Citas para examinaciones y evaluaciones pueden ser necesarias.  Tales citas, no seran necesariamente el dia de la matricula.
  11. Si hay algun problema que no se puede resolver, por favor haga una cita para discutir el problema con un miembro del personal de esta oficina al telefono 434-2301.
